A: Use the Case Information page to enter the details of the household and the benefit program(s) that you represent. You can select a single program or multiple programs that your allowed to view for the applicant you represent.
A: Click the Back button to return to the Authorized Representative Request page. The information you entered will not be saved.
A: Click the Save and Continue button to save your information. If you have documents to upload, you will go to the Verification Documents page, otherwise you will go to the Submit your Request page.
A: Click the Home hyperlink to return to the Home page.
